Intensity Hoops — to be read and agreed to by a parent or legal guardian on behalf of any participant under the age of 19, before participation in any Intensity Hoops program, practice, camp, clinic, tryout, or travel activity ("the Program"). This form covers assumption of risk and release of liability, emergency medical information and consent, and consent for photos and video. It does not cover registration fees, refunds, or cancellations — see our Refund Policy for that.
1. Waiver of Liability, Release & Assumption of Risk
Basketball is a physical activity that carries an inherent risk of injury, including but not limited to sprains, fractures, concussion, collision with other participants, and — in rare cases — serious or catastrophic injury. I confirm that my child's participation in the Program is voluntary, that I have had the opportunity to ask questions about the activities involved, and I knowingly and voluntarily assume all risks of participation, whether arising from the ordinary negligence of Intensity Hoops, its coaches, staff, or volunteers, or otherwise, except where caused by gross negligence or wilful misconduct.
In consideration of my child being permitted to participate, I, on behalf of myself, my child, and our respective heirs, executors, and assigns, release and forever discharge Intensity Hoops, its owners, coaches, employees, contractors, and volunteers from any and all claims, demands, or causes of action arising out of my child's participation, including claims arising from ordinary negligence, except to the extent such release is prohibited by the laws of British Columbia.
2. Emergency Medical Information & Consent
Allergies, medications, medical conditions, and anything else a coach or first responder should know are collected through the Medical Considerations field on the registration form, alongside the contact details already provided at registration. This information is kept confidential and used only for the participant's safety.
I authorize Intensity Hoops staff to administer basic first aid to the participant I am registering. In the event of a medical emergency where I cannot be reached within a reasonable time, I authorize Intensity Hoops staff to call 911 and to consent to emergency medical treatment, including transport to hospital, on my behalf until I or another contact provided at registration can be reached. I confirm that, to the best of my knowledge, my child is physically fit to participate, and I will keep the Medical Considerations information current and disclose anything new that could affect safe participation.
3. Media Consent
Intensity Hoops regularly photographs and records video at practices, games, camps, and events, and may use a participant's name, team, and image — on our website, social media, and promotional materials. We do not sell this content or license it to unrelated third parties, and we will not publish a participant's home address, school, or other identifying information beyond their name and team. By agreeing to this form, I consent to this use. This consent cannot be withdrawn.
4. Governing Law
This document is governed by the laws of the Province of British Columbia. If any portion is found unenforceable, the remainder shall continue in full force.
5. Acknowledgment
By checking the box for these terms during registration, I confirm that I have read this form in full, understand its contents, and am agreeing to it freely and voluntarily, on behalf of the participant I am registering. This acknowledgment is completed electronically as part of registration — no printed or wet-ink signature is required.
